Short Term Case Management for Young Parents and Pregnant Women

Provides services for pregnant women regardless of age. Services include a pre-natal risk assessment and linkages to resources. Staff will assist clients in completing applications for HUSKY medical coverage. Options counseling, crisis management, and supportive counseling is provided. Clients are seen in the office, however, home or community visits may be arranged. The program works collaboratively with medical and mental health providers to achieve healthy birth outcomes.
