How to file a complaint
It is your right to register a complaint. Registering a complaint will in no way compromise your care. In fact, it will enhance our communication and assist us in meeting your healthcare needs. Once you have registered a complaint, you can expect that the issue will be reviewed and you will receive a verbal or written response.
A complaint can be made in one of the following ways:
- Contact the department manager/supervisor.
- Contact the Quality Improvement office. This office is open Monday through Friday, 8:00 a.m. through 4:30 p.m. The phone number is (860) 358-6151.
If the office is closed, you may leave a voicemail and a Quality Improvement staff member will return your call the next working day.
- If your complaint does not require immediate attention, you may write to Robert Kiely, President & CEO, Middlesex Hospital, 28 Crescent Street, Middletown, CT 06457.
- You may also contact The Joint Commission’s Office of Quality Monitoring or the Connecticut Department of Public Health Complaint/Compliance Unit to directly report any concerns or register complaints.
