Behavioral Health Care
Provided by nurses certified in behavioral health nursing.
Diabetes Care and Education
Helps patients learn the self-management skills they need to manage their condition on a day-to-day basis.
Cardiac Care
Specialized cardiac care is available for cardiovascular conditions.
Heart Smart program
An integrated, multidisciplinary management program that includes well care, early recognition, medical intervention, patient tracking and counseling for patients with congestive heart failure and post-operative open heart surgery needs.
Community Health Services
Includes flu shots, health fairs, Meals on Wheels, transportation and adult day care.
Geriatric Care
Understanding the special needs of senior patients, including management of conditions, complex medications or long-term illness.
Infusion Therapy
Full-service infusion therapy at home for conditions such as parenteral nutrition, antibiotic therapy and pain management.
Lifeline Program
An emergency response program which triggers a response to the client from a support network based upon activation of a personal emergency button worn by the client and connected to the phone system. Call (860) 358-5406.
Maternal/Child Health Program
Comprehensive program that addresses the skilled nursing and case management needs of perinatal and pediatric patients and their mothers. Assists new mothers, provides lactation consulting, newborn care and children with special needs.
Medicare-certified Hospice and Palliative Care
Dedicated to enhancing the quality of life for terminally ill patients and their families, providing comfort with relief of physical symptoms as well as emotional and spiritual support. All while recognizing the patient’s right to make choices and remain as independent as possible during this phase of life.
Psychiatric Nursing
Management of patients with primary psychiatric illness living in the community.
Rehabilitation Services
Comprehensive medical rehabilitation for post-orthopedic surgery patients and individuals with disabilities resulting from musculoskeletal conditions, neurological conditions, stroke, and spinal cord injury.
Respiratory/Pulmonary Care
Care for health concerns including, but not limited to, asthma, pneumonia, recent thoracic surgery, tracheostomy and oxygen therapy provided.
Wound/Ostomy Care
Specially trained RN/Wound Care Specialist to case manage difficult-to-heal wounds.
