The Center for Chronic Care Management at 80 South Main Street houses Middlesex Hospital’s Disease Management programs. The Center offers programs that assist patients with a diagnosis of asthma, diabetes, and chronic heart failure to live well with their conditions. The Center also offers childhood obesity, and smoking cessation programs. Since 1999 the Center’s dedicated and specialized professional staff of nurses, counselors, dieticians and care managers has helped thousands to enjoy better lives and improved self health management as patients receive individualized care to meet their specific needs. Most of these valuable programs are free of charge.

 

The Center for Chronic Care Management is comprised of the following programs:

Little AIR – a pediatric asthma program especially tailored for the unique needs of children with asthma.

AIR Middlesex– an adult asthma care program.

Chronic Heart Failure Program– attends to patients in the hospital as well as those who have been discharged home with the diagnosis of heart failure.

Diabetes Programs – several components of care are available for diabetic patients including Medical Nutrition Therapy, Diabetes Education and one on one Diabetes Care Management.

FitForKids – a program designed to meet the needs of children with obesity and weight management issues. A counselor works with the child, family and medical provider to support and manage nutrition and lifestyle changes that enable life long healthy results.

Smoking Intervention Service – a smoking intervention counselor provides assistance to individuals and small groups to cope with their smoking addictions.

Eligibility: Patients must have a physician affiliated with Middlesex Hospital. If they do not already have a physician, the Center staff will assist patients in selecting one. All care managers work in cooperation with the patients’ physician to provide education and guidance to manage their chronic conditions. Patients with multiple diagnoses may be enrolled in more than one program.
